Controlled copy holders of the Manual have been assigned a Control Number and will be notified of changes via e-mail. If you are NOT a controlled copy holder and would like to be notified of future changes, contact Sherri Scheifele (sscheifele@labtesting.com) or Frank Peszka (fpeszka@labtesting.com) for a Control Number. Upon receipt of a control number, click on the Submit Control Form button, complete the form and submit it as an acknowledgement.
